A claim is made when ones’ policy reaches maturity or upon death of the policyholder. When the policy reaches maturity, the policyholder is supposed to visit our Head Office at Nakawa Business Park on Block A, Level 6 with his or her original copy of the policy document plus a valid National Identification Card. In case one lost his or her policy document, we require a police letter indicating the loss of the policy document, statutory declaration, and a copy of valid identification.
Upon the demise of the policyholder, the next of kin makes the claim by visiting us at Head office with the original policy document, Death Certificate of the policyholder, and National Identification cards for the deceased and the next of kin.
Within 5 working days, submission of all necessary documentation, a discharge voucher is shared with the client via email. On submission of fully filled discharge voucher, the payment process is concluded in the next 5 working days.

The Unit Trust is an open-ended investment which means clients have got access to their units whenever they feel the need. The redemption process is very simple whereby the client fills in a redemption form and submits either through email, WhatsApp, or physically to the office and the stipulated units are then transferred to the client’s bank account.
For Cash Management, one can redeem their investment at maturity of the fixed period.

You may remit your premiums using any of the different modes below:

- Labour Department forms 31, 35 and 87.
- Original medical treatment notes from the attending doctor indicating the nature and extent of injuries sustained.
- Original receipts for the medical bills incurred.
- Employee’s pay slips for the 3 months prior to the injury date.
- Certified copy of their staff ID card.
- Medically recommended number of days spent away from duty as a result of the injury (attach the relevant sick leave records).
- Police report in case of road traffic accidents.
- Claim form duly filled, signed and stamped.
- Original medical treatment notes from the attending doctor on nature and extent of the injuries sustained.
- Original receipts for the medical bills incurred.
- Payslips for the 3 months prior to the injury date.
- Certified copy of the employee’s staff ID card.
- Advice on the number of medically recommended days off duty as a result of the injuries (attach the relevant sick leave records).
- Police report in case of road traffic accidents.
- Claim form duly filled, signed and stamped (attach a detailed breakdown of the claimed amount)
- Insured’s detailed statement on circumstances of loss.
- Detailed police investigation report on the incident.
- All documents in support of the lost cash/items (e.g. receipts, payment vouchers, cash movement records etc.)
- Insured’s after loss audit report.
- Culprit’s employment details (incl. application letter, employment contract, dismissal letter, details of their referees etc.)
- Any other documents deemed necessary to support the claim.
- Claim form duly filled, signed, and stamped.
- A detailed statement on loss circumstances.
- At least 2 replacement quotations for an item of the same make and specifications.
- Copies of the purchase invoices and / LPOs for the lost/ damaged items.
- Pictures of the damaged items.
- Police report in case of theft.
- Claim form duly filled, signed and stamped.
- A detailed statement on circumstances of the loss.
- Police report on the incident/ fire brigade report.
- Replacement &/or repair quotes for the damaged items.
- Stock movement records (for loss or damage to stock of goods).
- Claim form duly filled, signed and stamped.
- Statement of the Insured on the incident and related proceedings.
- Your technician’s report on the cause and extent of damage including recommendations.
- Repair &/or replacement quotations (at least 2) from separate service providers.
- Photographs of damages.
- Maintenance records for the affected machine.

- Police investigation report on the incident.
- Insured’s detailed statement on the loss circumstances.
- Letter to the security company holding them liable for the loss (the same should be duly acknowledged received by them).

- Pictures of damaged vehicle clearly capturing the registration number and the damages.
- Valid driver’s permit.

- Police report if there were any incidents along the way to Uganda.
